Medical & Money Eligibility with your Application for Medicaid in Colorado

There are three steps to get started with Medicaid LTC home care in Colorado.

Step One: You Need to Medically Qualify

Check Your Eligibility Online

Denver created the best online eligibility guide, MyFriendBen. See what you may qualify for.

Doctor Validation Form (ULTC 100.2)

Your doctor validates your LTC eligibility using the ULTC 100.2 Health Form. The cover letter is from TRE; the rest is Colorado-wide.

Step Two: You Need Limited Income & Assets

You need limited income and assets, and you need to prove it with bank or financial statements, so keep all your receipts.

Income Limit

Income needs to be under $2,982/mo. If you make more, an Income Trust, Funeral Expense Trust, or VA Aid & Attendance can help lower your countable income.

Asset Limit

Assets under $2,000 (individual) or $3,000 (couple), plus one house (under ~$1.7M), one car, and under $2,000 combined across bank accounts, life insurance, burial, and investment accounts.

What You Need to Prove

Prove everything, it is real money paying for your care. Gather all statements for burial, income, life insurance, bank accounts, investments, and vehicles.

Spending Down Your Assets

Medicaid has a 5-year lookback, anything given away in the last 5 years counts. Pay off debt, complete home and car repairs, visit loved ones, and pay Fair Market Value to contractors. Review your mortgage options.

Phone Application

Call 800-221-3943 or use state relay 711. Note: you still need to provide all background documents, so this can take longer.

Other Options

Many organizations help with this, some free, some over $10,000. Do not double-apply; it works against you.
